Before a Cuddle Therapy session can take place a Comfort Agreement, T&C's and Client Intake Form must be read, completed and returned. They can be emailed to you to sign and return, or completed and submitted here.

Important note - please complete ALL THREE FORMS. You MUST read and sign the T&C's and Cuddle Agreement AND complete the Client Intake Form before a booking can be arranged.
